Look for the Winning Wednesdays logo (as seen) and send it to the Southern Health-Santé Sud (SH-SS) Regional Payroll office via fax, email, inter-departmental mail, or in person.  This is a fillable form – please complete it, sign it, and send it! OR if you prefer print a copy and fax, email or drop it in the interdepartmental mail.  Enrollment/Change/Cancel forms for subsequent draws must be received two weeks prior to the draw to be entered in to the next draw or to be removed from the next draw.  The amount of $5, $10 or $20 will be deducted from your paycheque, depending on what you select, Foundations 50/50 (or similar) will be the deduction code on your pay statement.

No. Once you have filled out the Enrollment/Change/Cancel form and sent it in, you are enrolled for every draw for the 2022-2024 LGCA license.  If you wish, you can submit the Enrollment/Change/Cancel form again indicating you would like to cancel your enrollment or change your ticket choice for the SH-SS Staff 50/50 Payroll Draw.  It does not take effect immediately. Please allow for the two week period for data entry.

Your ticket(s) will be emailed to your personal email address provided on the Tuesday before each draw.  Please be sure to fill out the form very clearly so that you receive your ticket(s).  Your ticket(s) will be emailed from the third party provider, Funding Change.  An email address in mandatory for enrollment.

The winner will be picked on the Wednesday following paydays, at 2:00pm.  The draw will take place in the BTHC Foundation Office in the Boundary Trails Health Centre, with the service provider Funding Change, using electronic randomization equipment, approved by Liquor, Gaming and Cannabis Authority (LGCA) of Manitoba.

The winner will be contacted by email and/or telephone.  The winner’s name will also be printed in Prompt and listed on each Foundation’s website and social media accounts.

Yes, you can fill out the Enrollment/Change/Cancel form and submit it to the SH-SS payroll office.  You will only be entered in the draws when you receive a paycheque.  If you have not worked a shift within a pay period, you will not have received earnings to have your ticket amount deducted, and will not be entered into that draw.  Also, if you retire, you do not have to send in a cancellations form.  Once you are no longer in the Payroll system you are automatically removed.

The three main hospital foundations in the SH-SS region are working together to raise funds for the three larger facilities (Boundary Trails Health Centre, Bethesda Regional Health Centre and Portage District General Hospital).  We are giving SH-SS employees the choice to pick which Foundation they would like their ticket purchase to support.

Half the proceeds goes to the winning SH-SS Staff member, the other half goes to the Foundations for the purchase of equipment to benefit patients requiring care across SH-SS.

The “Foundations” consist of the three main hospital foundations in SH-SS. BTHC Foundation, Bethesda Foundation and PDGH Foundation are each non-profit, volunteer-driven charitable organization whose mission is “To raise funds to support healthcare needs in the regional community”. Their fundraising efforts benefit all who are served within the Southern Health-Santé Sud region.

The BTHC Foundation is the organizer and LGCA license holder.

You will receive your winnings by cheque.  Arrangements can be made with the Executive Director of the BTHC Foundation (phone: 204-331-8808 ext.2) to pick your cheque up at BTHC, or have your cheque mailed to you.  Should a cheque not be cashed within six months from the draw date, the BTHC Foundation will add the unclaimed amount to the proceeds.

No, just like purchasing any other 50/50 ticket, you will not receive a tax receipt.
